A preliminary report by the Aircraft Accident Investigation Bureau (AAIB) has revealed that critical safety protocols were not followed in the crash of a Pawan Hans helicopter near Mayabunder in the Andaman & Nicobar Islands. The accident, which occurred on February 24, resulted in the death of one passenger and serious injuries to several others.
The investigation found that the helicopter's emergency flotation system was not activated during the approach. This oversight led to the helicopter crash-landing into the sea with its floats unarmed, causing it to topple on its side in shallow waters.
The AAIB has recommended that the Directorate General of Civil Aviation (DGCA) enforce stricter compliance with float activation procedures and conduct spot checks on commercial helicopter operators. Additionally, the crew was able to evacuate all passengers, three of whom were later hospitalized, though one succumbed to injuries.
