The incident occurred at the Catalyst Refiners plant in Institute as workers were preparing to shut down part of the facility. A chemical gas reaction involving nitric acid instantaneously overreacted, causing the issue.
Ames Goldsmith Corp., the plant's owner, expressed sorrow over the events and vowed to cooperate with ongoing investigations to determine the cause.
